St Clare Meadows Care Center is part of SSM Healthcare of St. Louis MO, one of the largest Catholic sponsored healthcare organizations in the country and affiliated with St. Clare Hospital and Health Services.

St. Clare Meadows Care Center is licensed by the Wisconsin Department of Health and Family Services and participates in both the Medicare and Medicaid program. A fully certified staff offers 24-hour nursing care, exercise therapy, inpatient and outpatient rehabilitation and sub acute services.

St. Clare Meadows organized activities, intergenerational programs and regular religious services, multiple room options and proximity to local physicians, caring and compassionate staff and even Rocky, the facility Golden Retriever, help St. Clare Meadows have a homelike and lively atmosphere.
