What is the St. Clare Health Care Foundation?

 

Our Vision:  The St. Clare Health Care Foundation will be a bridge between the St. Clare Campuses and the communities we serve, facilitating partnership and investment that will result in the highest quality, local health care facilities, technology and services; optimal health literacy; and access to health care for all of our neighbors.

 

Established in 2000, the St. Clare Health Care Foundation, Inc. (The Foundation) is a not-for-profit corporation under the laws of the State of Wisconsin and a tax-exempt organization under section 501(c)(3) of the Internal Revenue Code. 

 

Since its incorporation in 2000, the St. Clare Health Care Foundation has raised more than $2 million to support the capital needs, programs and services of St. Clare Hospital & Health Services and St. Clare Meadows Care Center and to enhance the quality and accessibility of health care for all who live in the communities served by St. Clare.

 

Gifts to the St. Clare Health Care Foundation improve the quality of health care services in the greater Baraboo/ Lake Delton/Wisconsin Dells area. Financial support from the community makes it possible for the Foundation to purchase vital, new pieces of medical equipment, renovate and add new health care facilities, assist those who are uninsured, and develop new health care and educational programs, services and initiatives. 

 

Current, outright gifts and planned, deferred gifts are welcomed. Contributions to the St. Clare Health Care Foundation, Inc. are tax deductible as provided by law. As with St. Clare Hospital & Health Services and St. Clare Meadows Care Center, the St. Clare Health Care Foundation is owned by SSM Health Care in St. Louis, Missouri.  However, all gifts made to the St. Clare Health Care Foundation remain in the greater Baraboo/Lake Delton/Wisconsin Dells area to serve area residents’ health care needs.

 

 

How is the St. Clare Health Care Foundation governed?

 

The St. Clare Health Care Foundation is governed by a board of directors comprised of business and civic leaders from Baraboo, Lake Delton and Wisconsin Dells.   

 

Why should I give to the St. Clare Health Care Foundation?

 

Gifts to the St. Clare Health Care Foundation improve the quality of health care services in the greater Baraboo/Lake Delton/Wisconsin Dells area.

 

The St. Clare Health Care Foundation developed the St. Clare Healing Garden at St. Clare Hospital and the St. Clare Hospice House on St. Clare Meadows’ grounds.

 

The Foundation recently coordinated fund drives to purchase over 40 pieces of new medical and therapeutic exercise equipment for St. Clare Hospital’s Rehabilitation Services Department and to purchase medical equipment for St. Clare Hospital’s Surgery/Endoscopy Department, St. Clare Meadows Care Center, and St. Clare Urgent Care Clinics in Lake Delton and Wisconsin Dells.

 

Through the generosity of its donors and the work of the Foundation’s endowment, the St. Clare Health Care Foundation has sponsored medical equipment for nearly all departments and facilities in Baraboo, Lake Delton and Wisconsin Dells that are associated with St. Clare Hospital & Health Services and St. Clare Meadows Care Center. 

 

In addition, the philanthropy of Foundation donors reaches well into the community through the support of programs to help our uninsured neighbors with their immediate and chronic health conditions.

 

In gratitude to the community for its support and in an effort to advance health and wellness, the Foundation developed the Pathways to Wellness Initiative. Through original programming, partnerships with other organizations and co-sponsorships, the Foundation reaches area residents of all ages. 

 

The Foundation provides funding to area organizations engaged in health and wellness activities through its Healthy Community Partnership Grants initiative.  These grants are funded by a portion of the annual earnings from the Foundation’s endowment.

 

 

How may I make a gift to the St. Clare Health Care Foundation?  

 

There are several ways to make a gift to the St. Clare Health Care Foundation, ways that will make a visible difference in the health care services of our community.  By giving directly to the St. Clare Health Care Foundation, you have the assurance and satisfaction that your gift will make a difference in health care quality and accessibility in the greater Baraboo/Lake Delton/Wisconsin Dells area – today and in the future.

 

  • Gifts to the Foundation  

You can make a gift right now that is designated for a particular Foundation project or fund.  If you do not choose to designate your gift for a particular fund or project, it is deposited into the Foundation’s general fund.  The Foundation’s Board of Directors designates general fund gifts to where the need is greatest, including medical equipment.

 

  • Endowment

You can give to the Foundation’s endowment, which is the Foundation’s permanent investment portfolio.  Gifts to the endowment can be made now, through a pledge, or through your will or other estate plan.  Such gifts provide a strong and ever-growing source of support for all aspects of St. Clare.  An endowment is a simple, yet meaningful way for a charitable organization to grow and better serve its customers: A donor makes a gift to the endowment, which is then invested to generate income.  Your original contribution is never spent: A percentage of the endowment’s earnings are used to help fulfill the organization’s mission.

 

In addition to covering one-half of the Foundation Director’s salary and benefits, the Foundation’s Board of Directors makes annual distributions from the endowment’s investment earnings to help purchase new medical equipment, renovate and add new facilities at St. Clare Hospital and St. Clare Meadows, assist the uninsured and under-served with their health care needs, develop new health care and educational programs and services, and partner with area organizations and businesses through a granting program.

 

As the endowment grows, so do the earnings from it. Therefore, your gift will help serve more health care needs far into the future. 

 

·        Golden Oak Legacy Society

 

The Golden Oak Legacy Society honors and recognizes friends of the St. Clare Health Care Foundation who have made a gift of $5,000 or greater to the Foundation’s endowment (including its named endowed funds – please see below) through a current gift, pledge or estate gift.  Those who join the Golden Oak Legacy Society may choose to remain anonymous or have their names added to the permanent wall of honor located in St. Clare Hospital’s main corridor across from the Foundation office.

 

  • Named Endowed Funds

Donors may restrict their gifts for the Foundation’s endowment by creating their own named endowed fund with a gift of $25,000 (an initial gift of $10,000 or more is required and the remainder must be given within a five-year period).  With a named endowed fund, you can designate to which Foundation fund or St. Clare department you would like your gift to provide lasting support.  As your contribution is preserved and invested, a portion of your endowed fund’s value is used for your designated purpose. Named endowed funds are open-ended for the later addition of gifts.  Other people can contribute to your named endowed fund.  Such additional gifts increase the amount of annual support that your designated purpose will receive.

 

The Foundation currently manages three named endowed funds within its endowment.  Any interested donor may contribute to these funds: 

 

  1. Baraboo Rural Residency Training Program Fund, which supports the program that helps alleviate the anticipated family practice physician shortage in the greater Baraboo/Wisconsin Dells/Lake Delton area by providing on-site training for family practice residents and interns in the field of rural medicine. 
  2. Faith Endowment for the Chemically Dependent & Co-Dependent, which supports services provided to the chemically dependent and co-dependent population through the St. Clare Center.
  3. Jean Diehm St. Clare Hospital Auxiliary Fund, which contributes to the Auxiliary’s annual support of medical equipment and other capital needs for St. Clare Hospital.             


How large is the St. Clare Health Care Foundation’s endowment?

 

The Foundation’s long-term goal is to invest, grow and serve the health care needs of the community through its endowment, its permanent investment portfolio. Despite market volatility and a slowing economy, the Foundation’s endowment’s value has remained fairly stable at more than $1.95 million. The Foundation’s endowment is invested and managed by AMCORE Wealth Management. The Foundation utilizes a 5% spending rate to make annual distributions from the endowment to fund projects that benefit local health care needs. 

 

 

To remember the St. Clare Health Care Foundation in your will or other estate plans, the legal name is St. Clare Health Care Foundation, Inc., located in Baraboo, Wisconsin.

 

Notice of Privacy Practices

 

We may use certain information (name, address, telephone number, dates of service, age and gender) to contact you in the future to raise money for St. Clare Hospital & Health Services.  We may also provide this name to our institutionally related foundation, St. Clare Health Care Foundation, Inc., for the same purpose.  The money raised will be used to expand and improve the services and programs we provide to the community.  If you do not wish to be contacted for fund-raising efforts, please notify the Foundation in writing to the following address:  St. Clare Health Care Foundation, Inc., 707 Fourteenth Street, Baraboo, WI 53913.
