Diabetes Education
 
Diabetes is a serious, common, costly, yet controllable disease affecting over 16 million people in the US, including over 2,000 people in the St. Clare Hospital service area.
 
People with diabetes are at risk for blindness, kidney disease, foot and leg amputations, heart disease and stroke.

Many complications can be prevented or delayed by aggressive preventive care, prompt identification of problems, and early intervention and treatment.
  • Have you just been diagnosed with diabetes?
  • Are your blood sugars out of control?
  • Do you need more information about diabetes?
  • Have you developed diabetes complications?

        The Baraboo Area Diabetes Program has individual and group sessions that provide information, guidance in making behavioral changes and methods for ongoing diabetes management. The program is recognized by the American Diabetes Association for Quality Self-Management Education. It is intended for people with Adult Type 1 and Adult Type 2 diabetes, provided in individual, group or combined settings.


        In individual or group classes you will learn:

  • What diabetes is
  • Nutrition information
  • Medication options
  • How to monitor blood sugars
  • Effects of stress on diabetes and stress management techniques
  • Available support systems and how they help
  • Acute and chronic complications
  • Foot, skin, and dental care
  • Health care and community resources
  • To put it all together
  • To make a personal treatment plan

DIABETES SUPPORT GROUP

Meets the 1st Thursday of each month from 6:30 to 8:00 p.m. Call Melanie Mielke, Diabetes Educator at 356-1510 for more information.


INSURANCE COVERAGE AND COST
  • Everyone is welcome.
  • A physician referral is needed.
  • Call your insurance carrier for prior authorization.
  • Medicare covers group sessions.
  • Individual sessions are covered by Medicare on a case by case basis.
